B is pretty close.

I suspect  the "wimpy shift" explanation covers it.  It's only happened
when I'm just clicking through the gears, not accelerating much.  I'll
concentrate on Form & see if it repeats.

A Easy.
B Generally easy but sometimes aggressive.
C Mostly hard and aggressive riding style.
If your answer tends to lean towards C, then
you might have bent or worn shift forks.
