The effects of this vow are primarily social, so I imagine that the
value would be based on the society, in particular how much grief he
is likely to get for following this vow.

For example, if every physician takes this vow, then he would get very
little grief and it would only hurt him when his allies are dying.

Other places he could be imprisoned for life for 'letting a man die'.
