First, make certain that when you have Lotus Notes open that you have the 
"NOTES" set file associated with the program.  Usually
you can hear which set file is associated by pressing CONTROL-Backslash when in 
Notes which brings up the Window-Eyes control
panel.  The associated "SET" file can be spoken as part of the window title.  
If that option is not set, arrow down through the
Window-Eyes Control Panel until you hear "Associations."  If that item is 
"closed" press the RIGHT ARROW to open it, then DOWN
ARROW to "Current Associations and tab through those boxes..  If the associated 
"SET" file is not NOTES, you may need to install
the set file from the factory sets.

If the problem you are having with the inaccurate reading of the message list 
happens after you have closed a message you have
been reading, you can generally fix this by moving focus away and back.  Press 
ALT-Tab, then release it, and then press it again
to get back to Lotus Notes.  You can also fix this staying within Lotus Notes 
by pressing SHIFT F6 and then F6 which moves focus
to another Notes Window.

If you are using Window-Eyes 7.5, it is worth upgrading to the latest version.  
I believe there was an issue with reading a line
multiple times a couple of versions back that was corrected, but I don't 
remember if it is the exact problem you are having or
not.

Finally, I had thought there were some issues with Lotus Notes 7 and Windows 7 
having nothing to do with Window-Eyes.  If others
are using it with Windows 7 in your work place, then I am either wrong or they 
have found a work-around, but if there is any
chance you are actually using Lotus Notes 8, there might be some other things 
to check.
