An update for anyone who is curious on this.  The project manager on the 
roll out of our Cisco Jabber instant Message client talked to Cisco, who 
admitted that Jabber was not set up for screen readers and they did not 
offer any suggestions.  Thanks to the information Steve Clower provided, 
the project manager has downloaded Miranda and is testing whether it will 
work in our corporation's technical environment.  Apparently Cisco largely 
developed the jabber protocol you all referred to, which is why their main 
product is called Jabber and the protocol is the same name.  The project 
manager was on vacation last week, so hopefully I find out this week his 
results.  Hopefully Miranda will work. 

As a matter of interest, I copied the head of our corporate medical 
department when I contacted the project manager about Miranda so that I 
would have her support in this as an ADA request.  Luckily the project 
manager didn't push back.  But copying the medical group also allowed them 
to keep ears out for other blind users in our company who might also have 
issues with Jabber.

In addition to the below, if your company is just using a front-end to a 
locally-hosted Jabber service, Miranda IM (discussed yesterday) also 
supports it. Since Miranda is accessible, using it in loo of a 
problematic Cisco implementation might work in your situation.
