Jamal, I found version 2.2 of the template which I have been working with, and the dialog seems to work correctly in this version. I am not sure that I have tried each version since 2.2, but I have tried some of them, and as far as I know, 2.2 is the only one which will bring up the sample dialog. It is not associated automatically, so use Script Manager to load it as a global script, then try Control-Windows-S to invoke the sample dialog.
