For something like this where there's not one good place to slice it
vertically, like you could do with a regular pair of pants, trace the
pattern onto another piece of paper and draw several vertical lines
(parallel with the grainline) at intervals across it. Perhaps every
3 or 4 inches. Then slice the paper on these lines and spread the
pieces apart on a larger piece of paper. Usually you would be
careful to keep them parallel, but if you need more room at the top
than the bottom, you may wish to angle them evenly so that they're
spread apart at the top but all touch at the bottom. Make sure that
the spaces between the pattern slices are all even at the top. Trace
out the new shape. Slash and spread the other pattern pieces to the
same degree (unless the waistband is a rectangle, in which case you
can just lengthen it). Make sure you transfer the grainline to the
new pattern pieces.

Finding fabric with enough stretch *and* some cotton content is a
chore from local sources, but the bigger problem (no pun intended)
is that I need to size it up. The pattern is basically two pieces,
plus the waistband. The main pattern piece is shaped like a fat
wishbone or Y.
What is the best way to add some width to the pieces? The wearer
will probably need additional room from calf to waist.
