Looking at my copy of Holbein in England from the Tate, which has a decent but still too small image of this one, it looks like there's just not enough detail in the sketch itself to say for sure. There's definitely something there, but it's just a sort of a v-shape where the lines meet, with shadow around it suggesting something that sticks out a bit. It could be a lump where they meet, though that's not the shape that 4-finished-seams-meeting makes when I do it; it could be a button, though the sketch suggests a diamond-shaped ...whatever... and a self-fabric diamond-shaped button seems a bit odd. It could be a covering patch, but in the sketch the ...thingy... is pretty 3D, and seems to stick out further than a patch would.
