It's a bit like trying to convince the average home owner in the early 1980's to buy a PC and a DARPA network connection for the purpose of exchanging information with others on the DARPA network. Large institutions came first and exchanged information between them. Even things like e-mail were largely driven by the equivalent of "hub and spoke" systems. (I consider myself a "spoke" in the early 80's.) Likewise, major health networks will first need to be in the habit of exchanging complete and partial health records electronically as a matter of everyday practice. At about the same time those same health networks will be providing similar access (and expectations) to smaller organizations and clinics. CMS will likely have a significant role in this as well.
