Or maybe even an RTS-style game like NS or the old C&C:Renegades
multiplayer, where you can choose to be a certain unit and just play
it out on a large battlefield. That would certainly take up more than
16 players per team if you consider a mixture of land, air, water
units, infantry, tanks, stationary defenses, etc.

But the real question is if the engine can handle the network traffic!

On a BSP engine you can make different separate areas linked by
corridors, or something like.

About RTS...

..You can make a GTA alike mod, "thug wars". Give money to a player to
buy weapons and hire street mercenenarys, maybe hire other players. Let
the players patrol the map competing for "spawn areas" somewhere like on
MMORPG, but in a no-nice way (fighting each others for the right to
loot). Make the mercs consume money, and quit if money become cero, so
you are forced to move and collect money for your personal army.

If the theme its ugly, you can swich to "necromancer wars", where you
spawn minions and upgrade the minions with mana. Patrolling the map to
capture souls, adquire mana sources, fight other teams, etc..

Other ideas:
 - loom interface, whats really funny to make magic with music
 - able a arbitray no of teams, so 1 good player can fight 4 bad players
teamed.
