i did something similar in household death (hl1) for rune spawns.

i made a little ingame editor where i just walk around to places where i'd
want runes to possibly spawn, hit a key and it would add a location, create
an ent so i can see where i'd been, at my feet
then when i was done, i wrote it out.

then when the map laods, it loads the file for the map, creates the rune
pedestals (you wouldn't really need to do that for regular items) and then
randomly spawns 1 of each rune at a random spawn point.
