It's so easy to deal with this problem.    It never happens on my servers
anymore.   The following is just one of many name changer plugins I run

my servers are on lock down through the use of well coded plugins.
the only thing you can get away with in my servers is playing the game and
having a ton of fun. period.
the only reason to have asshats in your servers at all,  is your either your
a lazy admin or the admin doesn't care about his regular clients.

An add on to this, how do you ban the right person when the hacker / jerk
changes his name to someone elses in game?  It's been a while for this to
happen to me but when it does I always get confused =)
