Regardless of whether or not it was user error, it's still relevant to server administration. This mailing list is here for server administrators to talk amongst each other to solve issues and answer questions as a community - it's not just an outlet to express frustration over valve-created issues that we have no control over.
It happens with mm and sm disabled.
