We have discussed this previously on the list. The hangs seems to be
occurring most frequently right after a map change while players are joining
the server. There is no crash (or crash dump), the server simply "hangs" and
needs a manual restart. There are no errors in the logs when it occurs.

It's been difficult to track down because it has occurred on every possible
server config (with and without sourcemod, 24 & 32 player servers, pure
vanilla configs, stock and custom maps, etc.). I am running Windows server
2008 R2 on (underloaded) dual quad core boxes with at least 24GB of ram.
