There's some kind of exploit/hack out that gives the user a permanent
pending steam id. This is more of a steam id system hack and you will need
to ban the user by IP address. IMHO anyone who's using this crack...is also
willing to hack in other ways and should be banned instantly.

rcon writeip <ip address>

Or add it manually to the ip ban list...sadly...if the user is on an isp
that has a very short or non-existent DHCP IP lease this will be difficult
to admin...I generally do not ban entire isps...but if it's an out of
country I may consider it.

I don't think the server checks the ID banlist before the player ID is
validated.
