+1 on slicehost - I've been working on a couple sites there for a while.
As Tom said, bare-bones, but they have a good collection of guides to  
get things set up.
Plus, there's a lot of good tutorials on their site to get things up  
and running.
Finally, there's a set of Capistrano tasks 
(http://github.com/josh/slicehost/tree/master 
) that
looks like it can automate most all of the setup stuff. Haven't used  
them, but they are
actively developed.
