If I remember correctly, the original Schmidt piston double was designed for a double jointed basketball player who could palm a basketball. Seriously though, I had one that had a hook, like a pinky finger hook, soldered on the left side of the horn that acted like a duck foot and carried the weight of the horn between the thumb and first finger. This allowed more flexibility to move the hand down to a more cramp-free position. It worked for me. Also, I believe someone has rigged a linkage between a more typical thumb lever shape/location to the piston button. Maybe a more informed list member can shed some light?
