I think you will find that a lot of people (including me) use Qimage and some additional number use a custom profile. I'm speaking for the PC crowd in this, I don't really have a good sense for how things are in Mac land.
I'd say that Qimage is a vast improvement over nothing, and the profile is a noticeable improvement - much more improvement for some images than others, certainly. As with most things, you get diminishing returns for the input of effort and cost into things like RIPs.
