We have 6 - HP 90's that run about 4 hrs every day and have for the last
several months. We have the same problem with yellow printheads. Not
every yellow printhead, but I would say most. When they do not last the
100% like the report says, we call HP and they send us new ones. The
printheads are not cheap and they are guaranteed.

Maybe ink, but if a new Cartridge doesn't solve it, look for 
hardware defects. 

Yellow is the first PH, LC is the last, might be headcrashes, what 
are you printing on? is it very thick? Otherwise check your carriage 
height (or have someone to do it).

The heads are prefilled, maybe you're just printing them "empty" 
without any ink coming through the tubes. Are the failed ones 
noticeably lighter than the other ones?
If so, check the tubes for air, if there are bubbles, mark their 
position with a pen, and check if they move when you print.
You're looking at a new ISS and, if you're at it, tubes.

If the PH aren't lighter, check the service station, the capping 
might be faulty.
Catch the Carriage when its underway by removing the power chord and 
check if the Service station, if there's anything abnormal visible 
(paper, dirt, the capping mech is skewed, stuff like that.

2 weeks of proper function seems too sporadic for an electronic 
electronic problem.
