We get around the frameset dilemma by using javascript.
We index our site normally... and each page header contains some javascript
to ensure that the page loads in it's proper frameset. A perl script called
gateway serves as the frameset page. It's not an all-encompassing perfect
solution but it works for us.
See http://www.altrec.com/detail/1312 (a non-framed page) will reload itself
into http://www.altrec.com/gateway/detail/1312

View the frame source of the bottom frame to see the javascript.

ht://Dig is rated as having "limited" search result customization
capability, which I would dispute.

Another feature MondoSearch has that's nice is categorization of search
results by site-defined categories. Has anyone else looked at this? Do a
search at http://www.newcity.com/newcity/ to see the method.

Just thinking out loud, I guess.
