As the missing source code problem is popping up now and then, I'm thinking 
that maybe it would be a good idea to include the source code (compressed) in 
the load module, like you could do when compiling REXX programs.  This would 
guarantee that you always have a source code matching the running programs.

I think that the performance implications would (in general) be small today.

Even IBM has source & object that don't match.

Anything over 1K programmes introduces major issues.

As much as I would like us all to be current with COBOL,
I would still be a proponent of attrition, rather than 'en masse'.
