Obviously the first two are, to some extent, possible for program objects using RMODE=SPLIT (but that sort of functionality is not available anywhere near early enough to be used for the nucleus. And of course RMODE=SPLIT
support was developed long after.

Presumably the extra processing needed to load the additional control
information and to process this stuff was deemed necessary for the nucleus
and not important enough to be accommodated afterwards.
