On what release of Z/os did this happen?

We had a catalog break a little yesterday.  First symptoms were an svc
dump and a cluster of messages like:

IEC331I CATALOG.MISC.UCAT

I sent the dump to IBM and they came back with:

Looking at your dump, it looks like a match for
UA33288, which is the PTF for OA19870.  To verify this, can you Examine
catalog CATALOG.MISC.UCAT.

Title for OA19870 is CATALOG IN ECS EXTENDS BUT ITS VVDS IS MISSING AN
EXTENT

ERROR DESCRIPTION:
  A catalog in ECS took a new extent but later errors were
  encountered on its re-open after an AUTOTUNE. Message
  IEC331I 026-008(21080018) was issued and catalog failed to
  open the catalog. EXAMINE showed errors IDC11728I DATA FOUND IN
  EMPTY CI

PROBLEM DESCRIPTION: Since enough documentation could not be
                         obtained to ascertain the root cause of
                         this problem, we have added diagnostic
                         checks that will detect and issue a
                         dump when a VVR that is about to be
                         written would break the catalog (5
                         different checks). Also the VVR I/O is
                         not performed, thereby preventing any
                         breakage.

PLEASE NOTE THIS APAR IS NOT MARKED HIPER.  I have addressed with with IBM
and they are looking
at why that might have happened.

Our Storage group worked with Dino Software and we were able to get the
catalog fixed.  We also disabled ECS
until we can get the fix installed
