output from the ACCEPT CHECK, I noticed that the OMVS ROOT HFS file was
not allocated.  I found this strange, as there was a number of PTFs that
modified this HFS file.

ACCEPT does not allocate, read, nor update target libraries, and the directories in the UNIX file system are considered a "target libraries" by SMP/E. ACCEPT updates the distribution libraries (dlibs) which are partition data sets (PDS). Therefore, you should not see the root HFS allocated for the ACCEPT operation.

Is USS maintenance not ACCEPT eligible?  I find this odd.  It begs the
question how does one back off USS maintenance?

"ACCEPT eligible?" Of course. The RESTORE command is used to back off PTFs, and RESTORE will copy members from the dlib PDSs into the UNIX file system.
