Try checking your ISPFLOG (ISPF LOG) dataset.  The easiest way is to split your 
screen and go to ISPF TEST function.  

Then Select 5 LOG and browse it.

If you did not start your ISPF Session with LOG setup, then do the following

1) Type Settings on the command line

2) Top Left should be LOG/LIST

3) Select LOG

4) Update it with about 100 pages.

5) Do not delete log when you exit ISPF for this

6) PF3 back to main menu

Now try ISMF again, and add your volumes.
When you get the error, then split screen and check your ISPF Log info from the 
ISPF TEST function.

See if you get any additional information.

Otherwise, you probably need to open a case with IBM DFSMS.
