I am running EMC disk array, formatted with 3390-3 disks. Some of the
information I saw below I don't see when I do a CPFMTXA format or ICKDSF
label. I did notice that you were using the INIT function which I don't
use.  You could try what I have above just to get the CP format with a
label and then try the ICKDSF INIT to get a VSE type VTOC written where
you want it and put a VSE type label on it.

The following message below I don't understand: ICK04000I DEVICE IS IN
SIMPLEX STATE
