I was thinking about this earlier, and my conclusion
is that Janice's farrier had a "sudden onset" that
caught her horse by surprise.  Unlike people with
chronic condiditons that the horse senses from the get
go.  Then the therapeutic horses have a knack and
compassion for disabled riders, just like some dogs do.
