It's usually caused by either a software or a hardware conflict of some sort that the system cannot resolve. Often, a particular peripheral may cause it. The good news is that generally, all it requires is a hard restart and you're back in business. But the bad news is that whatever caused the problem, especially if it's a piece of hardware, will likely do it again. So it takes some troubleshooting to figure out what would be causing it, but sometimes it may be obvious because you'll be doing something specific when it happens... I've only had one, and I was trying to open a corrupted gif file from a message in Entourage. The program immediately tried to start classic, which I didn't want to do, so I hit the "stop" button.
