in a situation like this, when a drive has gone south twice, i do a 
Zero All Data.

use the OS 9 CD. use Drive Setup. highlite your drive, go to the 
Functions Menu and choose "Initialization Options". check the Zero All 
Data option, then OK. in the main DSU window, hit initialize.

if this happens again, there's probably something wrong with the drive.
