One thing you could try if you haven't already is turn down your main output on your keyboard and turn up the volume slider in itunes and even the gain in your eq setting and see if that at least buries the hiss when your listening to music.
Maybe its an extenal device thats plugged in to the mac causing interference ?
Or bad shielding of the sound card from other components inside ?
Eventhow the intenal sound cards in macs aren't the highest quality, their shouldn't be audible hiss at moderate levels, that makes me think its bad shielding inside.
I'll do some unplugging of external devices and see if I come up with any thing.

I've found that my little pro-speakers drop in and out slightly <not totally but output has a slightly 'patchy' volume variation). It only happens below a certain volume however, and If i thurn it up to what would be my normal listening level it ceases to be an issue. I don't have any noticeable hiss, however.
