I do believe that you've got at least 2 separate formatting errors going on, yet the one to get those that we're taken on the DU used more than likely won't be found anywhere, the way to format the disk(floppy) will be done solely with the in-camera's formatting, anything else prolly won't do. ymmv Simply do all formatting and erasing/deleting in-camera and you should be good to go at all times.
