If the card comes with instructions, it will say. But, should it not,
the antenna plugs into the card, which gets plugged into the adapter,
which gets plugged into the computer. The antenna should be tethered
down near the RAM slots.

The iMac, if it is AirPort capable, will have an antenna. I myself
have a slot lead ruby iMac SE DV, with an AirPort card, and have not
had any problems. I don't know about any need of a fan, but would
