Hi Terry,
My wife's G4 has been having the same issue. I read something on the
Apple discussion site that you need to make sure the Helvetica font is
turned on. I imagine it is on most computers but it might be worth
checking. Not sure if duplicate fonts can cause this problem.
Resolving the duplicates did not fix the problem on the G4 but I
thought I would mention it.

I checked and my Helvetica font is turned on. I'm at my wits'
end with this and I think I'll try AppleCare to see if they have any
ideas.
To recap, I've
repaired permissions
downloaded Pacifist and tried to reinstall just the address book
trashed all preferences associated with the address book
done an "archive and reinstall" of OS X from my discs that came with
the machine.
Setting up a new account on the machine yields a functioning address
book, but I have yet to figure out a way to get access to all my
other stuff from this new account.
