That is most definitely a valid concern. We police the usage strictly to avoid these problems. We had a minor problem with one company about 4 years ago (not their fault as they had double opt-in procedures), but haven't had any since. We work closely with the customer to make absolutely sure that there lists are clearly opt-in, and that their content will not be construed as spam by the vast majority of their intended recipients (there are always exceptions). We have to, as we could not afford a problem with one customer affecting others.
Don't forget that even running this on a separate IP may not be enough to keep other IPs clean. Some blacklists will list entire ranges, so if you want to focus less on maintaining clean delivery, and more on avoiding problems with other IPs you will likely need a separate subnet, or even a separate provider entirely.
