You can (and should) still exclude any IP's that you do not know about.
Those that log in through other portals can use your server. They must set
up SMTP authentication... This is usually as simple for them as checking a
box and using the defaults. The box usually says something like "My SMTP
server requires authentication"

Servers that you know about by IP go right in.
Clients must authenticate - which is no big deal.
To be consistent, we tell everybody that they must authenticate their
SMTP... this keeps our client support simple because everybody is on the
same page - even if they're dialing in to our network directly.

This is what we do.
