Is it only happening when there are multiple recipients? Are you getting a bounce
message? What does the bounce message say?
It may be related to:

It looks like your mail server is listed in RBL and ORBS. With ORBS, you don't have
much to worry about. But, being listed in RBL will cause a lot of your E-mail to
disappear.
