Depending on the server, this should work fine. I'm not sure with IMail, but
many universities used to do this with their Unix/Sendmail servers. That way
they could also take down the server for maintenance and not lose any
important mail.
To make this work, you need to set up the secondary mail server to allow
ETRN requests from your mail server. Then just have your mail server send an
ETRN request every so often to make sure that no mail is accidentally
trapped there (i.e.: your router died and you didn't get mail for a couple
of minutes, etc.)
ETRN works off the SMTP port, not the POP port. This means that POP should
not interfere at all with ETRN. When you fire the ETRN to the second server,
it will attempt to deliver the mail to the highest priority (lowest number)
server.

I would like to offer a bullet-proof mail service to high end customers. I'm
running BGP4 but my building could burn down. Would the following scenario

IN MX 20 backup.myisp.com.
backupmail IN A x.x.x.x
I would run backup.myisp.com and backupmail.importantdomain.com at a remote
location using DSL.
If my ISP burned down, backupmail.importantdomain.com would make a ETRN
connection to backup.myisp.com and get the mail. My client would then
connect to backupmail.importantdomain.com to get their mail.
Wouldn't this work ?
Any way to make the ETRN and POP machine run on a single computer?
