I have been following your saga, appreciate all the time you are putting
into this and hope that there is a solution.
How do I know that we don't have any invisible messages? This is a small
company with less than 75 users. Most of these users are on the road at
least 50% of the time. When they are on the road they access their email
through Web mail. The users who have found they have invisible mail on
occasion now check every day or so went they are at the office to see if
they have it again. Believe me if they had any they would let me know, they
are nice about it but it upsets them that they may miss an important email
and they don't like having to check Web mail when they are at the office.
They want to look in one place, where their address book is and all their
contacts, not on the Web which doesn't have all the conveniences.
We have always had POP3 for the office computers so it is not something new.
Invisible mail is an intermittent problem here and doesn't effect all
accounts.

I've focused on Outlook Express because that's what I mostly work with;
however, it doesn't surprise me that Outlook should have the
same problem. I'm sure most of the core code is the same in both versions.
I am surprised that you said you had the problem with POP3 accounts. I have
been testing various configurations and did not
experience the problem with POP3. I wonder if the reason you don't see
invisible messages (no pun intended) anymore is because
you're using POP3. Or perhaps this was a bug that originally affected both
POP3 and IMAP, and then was later fixed, but for POP3
only.
I also wonder how you know that you aren't suffering from invisible messages
any more. The whole problem is that there are no error
messages or warning signs. Mail just doesn't arrive. If you have anyone
using IMAP, I suggest you look at their mailbox, because
you may be in for a surprise.
