yes

yes, for incoming, you have to put all the ip+hostnames in hosts 
file.  This is both "accept for @recipientdomain" plus "route 
@recipientdomain mail to ip" setting.

no, hosts file handles incoming mail, "relay for addresses" opens the for 
outgoing mail.
