It shouldn't be the result of a hacker.

You can ask their tech the IP address of the mailserver that sent the 
E-mail (and the IP address they sent it to, to make sure it was sent to 
your mailserver).  You can then search your logs for that IP, in case of 
some odd problem (for example, if they sent to a misspelled address).

It is technically possible that it could be a hacker (for example, if they 
added an entry to the HOSTS file of the server the mail was being sent 
from).  If that is the case, though, you should be able to tell from their 
logs what happened (if the HOSTS file was used, for example, you'll see 
that the E-mail was sent to a bogus IP).
