In most cases, the E-mails are being sent twice.  In some rare cases, 
duplicate E-mails will occur if the TCP connection is cut off after the 
E-mail is sent to your server, but before the remote mailserver receives 
the confirmation that the E-mail was accepted.

The best way to troubleshoot it is to check your SMTP log files, which will 
show whether the E-mail came in once or twice, and where it went.
