I think I begin to see your issue. I assume you are talking about some sort of server script and not another client tray gizmo.
The simplest thing would be to write a perl script that reads a list of mailboxes, acquired the passwords from the registry (or odbc), pop the box for a message list, notify if rule satisfied, loop until done. But there are so many places this thing could get so complicated. I think it is one of those seemingly simple things that might end up being an overly complex solution for what is really a very simple problem.
