Yes we have our internal mail going through an exchange server and the
internet email goes through the separate imail server.

I believe that's an Exchange thing, when it can't find a way to send out

Internet mail via SMTP.  Most likely, Outlook is misconfigured (such as 
being set up to use an Exchange server, when it shouldn't).
