In my opinion, content filtering works with ascii emails, but as more and more move to html, you need to get more aggressive as well.
This includes html option, spam databases mail from responses, helo responses etc. Here are some of the phrases I have added in, but I caution you I have a very extensive white list. You might trap a few unsuspected individuals otherwise.
