Sounds like a corrupt SAM, frankly. W2K AD and non-AD SAM work fine for us. Try performing a dummy install on a workstation for comparison; you could also migrate the machine to AD in its own forest for testing (this isn't such a terrible thing to have in prod, either, since you can then use AD for your address book).
Ouch.. We don't have this box on any domain or anything as this is a stand-alone server. We use IIS with FTP and other services fine with logins. What you are saying, is that we need to do a dcpromo to get this box in AD?
