It is my understanding that a DNS server will respond to an MX query with all MX entries, and it is up to the user-agent or relay host to decide what order to try the servers. Lower numbers come first, and more than one host with the same priority should be selected from randomly, if I understand correctly.
I have seen DNS servers cycle the order that the MX records are given, so there might be user agents or relay hosts that do not follow these rules.
