CPU processing

What you want to look at is the CPU usage when the server is at 100% -- if "Idle" shows up as 45% to 80%, that means that the server is using less than 50% of the CPU (so you probably aren't experiencing the problem with high CPU usage).
Quemgr, SMTPd32, and POP3d32 all rotate for the second most used process.
If that is the case when the system is at or near 100% CPU usage, it would sound like you are processing a very high volume of E-mail. But that doesn't correspond with the stats you gave -- a server with dual P3 600's should be able to easily scan well over 100,000 E-mails/day, complete with virus and spam scanning.
When you next experience 100% CPU usage for an extended period of time, you may want to check the IMail SMTP log file for an hour or so of that time (perhaps all entries between 1PM and 2PM, which would be easy to process) and count how much mail is being processed during that time period, to see if there is an abnormally high spike in the volume of E-mail being processed at those times.
