Are you using any spam filtering? Rules to delete e-mail? Declude? Something tickles in my brain about Outlook 2003 failing a command space test, maybe adding the return receipt alters that to pass some test? I'm totally guessing here.....but you did say any ideas.
Why not have her send and e-mail to herself with and without the receipt and see if there is anything different in the headers.
