Agreeing with Rick...

Certain ip4r/rhsbl improve and others slip with time. These wouldn't hurt to be 
adjusted.
If you have the Pro version the SURBL filter discussed a couple of months ago is 
pretty effective.
If you don't want to make adjustments much, Sort Monster's Message Sniffer usually 
detects 95+% of the spam with low false posives.

I change and update my filters almost daily to keep up with the spammers,
perhaps the fact that you havent changed your config files is the problem...
sorta like keeping your virus definitions up to date.
