I'll go obvious first. Do you have the Pro version of Declude Junkmail that is needed 
to run filters?

It looks fine. If you are generating the surbl.txt file the cmd file is working.

Anything unusual in the log?

If I had to change something, I might comment out the line that
TESTSFAILED END ISBLANK
