Right click on the toolbar and select customize. Go to the toolbars tab and highlight the Standard toolbar. Click the Reset button and that should fix your problem.
Just realized that I made one mistake, the toolbar in question is likely NOT the Standard toolbar. To find out which one is actually the culprit, undock the toolbar (drag it off to a floating menu) and reset the toolbar with the same name as the floating menu.
